Query related to form creation and updated using Jira SDK Development plugin

HI Team,
I have created Approval tab. Once you click on approval tab, a dialog box will open and this dialog will contain certain fields. Both Approval tab and dialog box have been developed using Jira sdk plugin.
Functionality :- I want to save the information added in dialog box in JIRA database once I click on Approval button. Below is the screenshot for the same.
1) How can I create this field in JIRA database and how can we map these fields values with the backend?
2) How can we create event for approval button to handle save functionality ?
