I have created a new Issue Type. I now have "Sub Task" and "Defect". The problem is that when I click on the create subtask button (the box with a checkbox) next to add attachment, - it creates a subtask. I know that you can click the new subtask button and then change the issue to Defect, but I wanted a quicker and more efficient solution. Therefore:
1. Is there a way to create a button that is: 'Create Issue Type 'Defect'' rather than Issue Type 'General'?
2. Is there a way to reorganise the buttons on the layout easily? e.g. having 'add attachments' to the right or bottom?
3. Can the these setting be set per user and/or per project?

So unless you have created a custom issue type and named it "Sub Task" then that is not possible. "Sub-task" is a special issue type that can only be created from within a parent issue not via the "+" button. If you did create a custom "Sub Task" issuetype I would first recommend against doing so as it will become confusing quickly (IMO). However, if your needs justify this then simply understand that it will behave like any other issuetype like Task, Story, etc. and when you click the "+" it will be a option for the project that associates that type. You can rearrange the layout in the new issue view by going to project settings > issue layout.
