Need to pull data of Jira Forms From Particular Dates

Hello 

We are Using Jira forms for 1 of the project

Here is my questions

1. When we pull the date to excel from Form from 3dots of Forms its downloading all the tickets along Forms. But, I required from particular date to particular date Is it possible then how can I download.

In each ticket we have 5 to 6 Forms available.

2. Buy JQL is it possible to download the reports with all attached Forms in tickets.
