Make field with options to be available to tick

Hello,

Please check attachment - is it possible with Inisght help to create such field with functionality like this?

There are several Data sources added where you would have possibility to check two options : "Requested" and "Delivered", if both fields are checked, you would be able to select date from calendar which is under "Date of delivery" option.

Would that or something similar be possible?
