Launching of Defect Template

I am looking for a configuration which can help me launch a jira defect template

Scenario: On the project dashboard we have different columns i.e.

Planned, In-Dev, In-QA, In-UAT, In-Prod

In case if i want to move back the user story from QA to Dev column I would want the defect template to launch for me to log the issue or the reason for this backward movement of the user story.

Secondly, If the above configuration is possible then is it also possible to have this template linked to the user story as a pre-defined value in the template 

Please confirm? and help me where should i look for the configuration
