Jira Issue Collector Not Allowing Me to Add Fields

I have 4 issue types, 3 system (Epic, Task, Story), 1 custom (Request) in my jira center project. I am trying to create an issue collector for Task but when it comes to the Issue collector form and selecting a template, I select 'custom' but for epic, task and story, its blank, for request, I see the fields I want. But I need to use the Task issue type.

As a Jira user when creating issues, i can see the fields on all issue types, but the issue lays when creating the issue collector.

Does anyone know what the issue could be?

The fix seems to be creating an issue collector using a basic template with the task issue type, then creating a 2nd with the same issue type which then allows me to add the fields.
