JIRa issue not getting delted - Throwing SQL error

While creating a jira issue, Duplicate tickets got created and they are not getting deleted and showing DB errors as below. Please help to resolve this issue.

There could be a number of reasons this is happening, but for us to narrow the list down usefully, you'll need to give us more of the error than a half-cut off screenshot - the first three lines and the last 20-30 in the log are the most helpful.
