JIRA Internal Directory users (not LDAP) are being inactivated

Background:  our system has both LDAP and JIRA Internal Directory users

For the Internal Directory users, we will manually inactivate their user ids for lack of use, but there have been a few that we've reactivated and we're experiencing some being set back to inactive.  In one case I activated yesterday and today it is back to inactive.

Typically we inactivate it simply by turning off the Active checkbox when editing the user record.

In some cases we're simply clicking the box back on to reactivate the id and for some I've removed all their user groups, reactivated the check box and re-added the groups back in.  

I've looked through the audit log and there is no entry.  Attached is one example from the log.
