Issue type scheme not working - scheme is associated to project

A new issue type was created in an existing project and the default scheme associated to the project was assigned to the issue type.  When I look at the configuration screen, it looks like the issue is configured correctly, however when I pull the issue up on the project board, the fields do not match the configuration.  

Other issue types in this project that are assigned to the same default scheme associated to the project and they pull up correctly on the project board.

To test this out in another project, I created the same issue type (Tech Debt) and configured it to use the same scheme as in the project above.  I associated the scheme to the new project and am having the same results.

Thoughts?

Thank you for the response.  Yes, I can confirm that all of these steps have been taken.  I have attached images to show the settings.  Please let me know if I have missed a step.

@Mohamed Riza - Any other suggestions of things I can try?  Or thought on if my set up is correct?
