Issue comment converts into attachment

a colleague commented an issue (task), but the comment was not displayed in the comment-section below. Later, he realized that his comment was automatically converted into an attachment. How is that possible? Maybe he used a special character at the beginning of his comment, that caused this behaviour?

There are two possibilities that could cause a new attachment to be added:

1 - Your colleague accidentally clicked on the attachment Icon in the formatting bar:

2 - Your colleague added the attachment name between two exclamation points (!!):
