Issue collector on confluence page

Hello,

In one project I prepared issue collector for some event registration. When I created issue from Jira project - I have screen that I want, but on issue collector template I couldn't add the same fields. I'm pretty new at Jira and I'm trying to find a solution because I want to add that collector to confluence page to register the event directly. I need "Reporter" field (which is system field) and "1st Reviewer" field (which is custom field) to add to issue collector.

but I can't add necessary fields to issue collector template nor via script to confluence page. 

Anyone can help with that?
