Issue collector form custom fields not applying

Those new to the Atlassian Community have posted less than three times. Give them a warm welcome!
See below. I select Custom Template, Select the Description field and update. But description field doesn't show up in the form.
Also - when I go back to edit the collector, the custom fields are no longer selected.
Bug?
