I've embedded an issue collector but sometimes when a user goes to the form it only shows the Name and Email fields and none of the other fields I chose to have visible are there.
I've found that if I jump over to Jira and just by going to the Project page under Jira Administration then go back to my page where the issue collector is the form will populate with all of the fields it should have. This is by no means a solution but just something I noticed that fixes it temporarily.
