Issue Adding Custom Fields to Issue Collector Template

Those new to the Atlassian Community have posted less than three times. Give them a warm welcome!
When I add custom fields to the issue collector template (doesn't matter which ones) the preview renders properly in the edit screen and tells me the update was successful.
However, the preview does not render when I click to view details on said issue collector, and the form url renders only a blank white screen
EDIT: when checking the sources downloaded you can see that the module-keys all read undefined
