Is there way to display issue create screen from workflow transition button?

I want to make an another issue (Issue B) from issue A when users click a button(ex. new issue) on the screen. If users can select destination project as like they click "create" button, it will be Perfect.

Process is like below. 

1. Users click "Make Change Issue" on issue A (issue type is "Incident")

2. Users can select a destination project of the issue B (issue type is "Change")

3. Issue A and issue B are linked (change - incident)

I want to use ScriptRunner and I don't use JMWE, also I don't  want to use an automation.
