Is there any feedback (survey) plug-ins for jira datacenter

We are currently using Qualtrics to send survey and then record in Jira but is there any plugin for jira datacenter to send custom form to submit the feedback and then record in Jira?

Finally, sending custom email, dashboards or reports available?

Any suggestions would be much appreciated.
