Is it possible to put instructions on how to use a particular project board?

I want to set up a board for people to post requests but I want them to be consistent and follow a certain format.
e.g. Name of the hospital site in the Header of the request. Name and email of relevant staff in the description.
Where on the board can I put this so that people posting requests can see it?
