Is it possible to create a RFC form for customers which creates stories in our Jira Cloud

Hi 

We are small software company that uses Jira Cloud as our internal task management system. The product we develop tends to have many change request (RFC - Request for change) from our customers. Does Jira Cloud or Atlassian have solutions where we can setup an online RFC form which integrates with our Jira cloud so that once customer submit an RFC a new story will be created with the description that was submitted in the form?
