In a subtask, how can I create a survey?

Hello,

I need to create a survey inside JIRA. But I need it to be in a subtask. How can I do that?

When I create the survey, is it possible to get reports about it?

I beleive the following addon might help you!

https://marketplace.atlassian.com/plugins/com.atlassian.jira.plugins.jira-survey-plugin

You might be able to pull reports using the custom field created by the survey plugin.

I don't have an answer, but I'm very curious: What do you mean by survey, or what plugin are you using that adds this functionality?
