I wan to create the button, but not able to get a correct redirection to my custom page, Please help

Screenshot 2024-10-09 at 4.06.40 PM.png I want to create a form sort of thing here as a jira plugin, what should be the redirection link for this kind of page to begin with, later I can just add my requirements in velocity template. But I am struggling to get the beginning right

The screenshot you are sharing is from Cloud, Cloud doesn't have velocity templates.

In my knowledge this will only be from a developed app. not in velocity templates.

You could ask your question in the developer community.
