I need to create a Jira page that allows people to leave feedback.

First off, I am a total newb at Jira; so bear with me.

My company has many internal Jira sites that we use. I want to create a Jira page that has form fields that a user can fill out.  Once this data is filled out, I need to be able to export the data into CSV format.  

Are there any tutorials or anything on this?

Jira is an issue tracker, so it "has form fields that a user can fill out" - that's what "create issue" does!  And you can export the issues to CSV with all the entered fields.

I would use the tools you already have!  Set up a Jira project to capture feedback, add the fields you want to it, and if you want to be a bit more clever, use an issue collector to inject a feedback link into your application(s)!
