How to submit a form which creates an Epic and child work items?

After browsing through forums it seems this should be possible, but I have created a form which upon submission will create an epic. 

I want to then use automations to create child work items under that epic. 

The problem is, based on what I currently have, is the child work item gets created but doesn't have the epic as the parent. 

When I attempt to set the epic as the triggering work item, it doesn't seem to go through. Is there something I'm doing wrong?

Hello @Anthony Carluccio 

The probleme is from the last action “Edit issue” is applied to your Epic, not to the last created Task.
To fix this, in the “Create issue” action you can set the field Parent = Trigger issue.

This did work, thank you! I'm not sure why but I tried this configuration first and the child issue wasn't being created.
