How to route a user to a Jira form if a field value matches requirement

I'm trying to figure out how to link one Jira Form with another based on a conditional rule. For example, if

1. A user fills out a form with specific mandatory fields, others being option
2. An optional field in the first form is populated
3. Once the form has been submitted, a Story is created
4. The user is directed to the next Form to create the next issue

So far what I have looks like this:

But it's the last part I'm not sure about - it says successful, but I don't land on the new form. Any suggestions?
