We want to use Jira Ops but - how to add incident templates from Ops to Jira Serve Desk customer portal?
What we want to get - to give a users an ability to inform Service Desk agents about troubles via portal and e-mail.
In the portal you can always have a request type called incident. You can setup the form to collect similar information as to what is collected in a JIRA OPS incident. Once this comes in you could use this to create a linked issue in service desk and link it with causes so it is updated when the incident is resolved.
