How to find transition ID by name using Rest API

Dear all,

I am using script runner and Jira API in order to proess workflow transition.

I am using the Issue transition API to transition the WF and it works well by passing the ID of the transition directly.

But in order to avoid potential change of ID, is tehre a way to find the ID of a transition by name from a given issue ?

if yes does anyone have a sample I could not get the proper way
