Thank you for the solution! I tried and it worked after a new task is created and being saved. But I would like the required items to be seen once I click on "+Create" and open a new page. Is there a way to do that?

Instead of Description field, you can use the "Custom field type" message and it should help show the message.

Thank you! This is great know, but once I added those as custom field, how can I bring them into "Description" where I want to use as a standard format.

Hello, we cannot add them into description field, you can add this message field below the description field. Or you can create a custom text field names as description and use that with default value. thanks!

Yes, it is quite similar with mine. But I didn't see a direct answer to meet my requirement.
