How to arrange fields in tabular form in the screen?

Hi all,

Is it possible to arrange 2 multi-lines fields side by side in a form or screen?

Or perhaps add a table with 2 columns and at least 3 rows?

My objective is when users are creating a service request, I want them to input the contact persons and their contact numbers. I can see that in the Description field, there is an option to insert a table, but I want to have pre-defined column headings.

I also saw a lot of suggestion to use a paid add-on for that table. But no free add-on.

Not in a screen, but you can do it with a Proforma Form.
