How to add fields Team and Sprint to the workflow to create Jira user story from Slack

I have created the workflow in Slack to create the Jira user story. However I can't add fields Team and Sprint to the form. Please let me know how to add those. Please explain how to configure what can be added as a questions at the workflow selection. Thanks!

Are the fields Team and Sprint on the create screen of the work type you want to create?
