At the moment I'm using an OnDemand Jira/Agile solution and I have created a lot of custom fields. Is it possible to export these custom fields to another JIRA instance, which is hosted within my company and import these custom fields?

Yes, this is possible.

Export your data into XML and import this file into your own hosted JIRA system. All unknown fields are created automatically as new custom fields.

The importer does it for you ;-)
