How do you create an intake form under a JIRA project?

I am looking for a way to have requestors of Ad hocs fill out a form under my JIRA project. Is this possible? Is there a way to send them a link and have it automatically go under my JIRA project?

@Casey Moeslein You can use an issue collector to get information from individuals and create issues in Jira Software without having access to the project.  The issue collector does have to be added to a web page.
