How can you generate a report with Proof of Test Evidence as well as Pass/Fail?

Looking for a report that includes all Proof of Testing evidence along with test case status of Pass/Fail

How are you storing these cases and their status?

Store them in their own project creating each as an issue using components to group individual projects and the Fix/Version to group by Release they are aligned to.
