How can I create an approval screen at the Release/Version level?

Try to create several transitions and use version as condition for each. To do this you could use script condition provided by ScriptRunner Plugin

Yes.  The Release view currently lists all types as a summary of the release and has the button at the top right to actually mark the entire package as 'Released'.  I need a form at this level to capture approvals for the entire release for audit requirements.
