Getting "/jira failed with the error “dispatch_unknown_error”" When Trying to Use Command in Slack

I've successfully installed JIRA Cloud for Slack App, but when i type /jira create in a slack channel i receive the following message "/jira failed with the error “dispatch_unknown_error”". Not sure what could be wrong here and would appreciate some help. Thanks
