Flexible Survey Tools

Does anybody have a good suggestion for flexible and customizable JSM survey plug-ins? The native functionality is on/off, and the plug-ins don't seem much more customizable. I'd like to:

• Define the email address from which surveys are sent
• Accept a response via email and/or link to the portal
• Define my own questions

You may want to take a look at the apps in the Marketplace, there are a number of survey apps, and I bet at least one of them can match your requirements.
