Hi, I have a Structure (Data Center) that allows manual adjustments. There is an issue on the Structure that should not be there based on the generators and issue's field values. The issue has the hand icon next to it. I assume it was manually inserted. Are there other ways it could have been added? How can I verify that it was manually added and figure out who inserted it.

The 'hand' icon on the left of an issue indeed means that it was manually adjusted. There is no history of such changes; they are not being tracked due to the architectural complexity of it. It's impossible to find out who, when, and why moved any item in the structure this way. This is one of the reason why we recommend to only use this feature when absolutely necessary to avoid potential confusion among users. In our experience, most use-case in structures can be realized without the use of manual adjustments - structures can be reconfigured or rebuilt to give the same output as manual adjustments can, but without breaking the consistency, as well as the rules of how the structure is built.
You can always disable the feature in each structure where it was enabled, and all made manual adjustments will be reverted. For this, you need to open the structure's configuration at Structure | Manage Structures and unselect the 'Allow manual adjustments of generated content' option.

Potentially, you could check this via the database and the structure-related table, but I haven't browsed through that one myself, so I cannot be sure.
My recommendation would be to reach out to Tempo (vendor) and ask them if they can provide some additional insights and exact table names for this. That's if something like that actually exists 👀
