Fields are showing up twice while creating a new issue

This is a company managed project and while creating any issue we can see the below fields appearing multiple times -

Thank you for your input, but that did not solve the issue.
Also, I am getting this only for "Company managed project". For "Team managed project" it is showing up once as expected.

Check if same field is created as Custom field and added in Company Managed project context.
