I have deduced that you are using the Advanced Forms feature in the project. is that correct?
Including here the message you are receiving:
This message indicates that the form is associated with the Task work item type.
The other part of this message is indicating that you have specified the field named Labels as a Required field in the Field Configuration you have associated to the Task work item type.
Because you have set the Labels field as Required for the Task work item type, your form must include a field that is linked to the Labels work item field so that users of your form can provide the required information. This will be true for any field that you have set as Required in the Field Configuration that is associated with the Task work item type in this project.
You can review the Field Configuration associated with the Task work item type to see which fields have been set to Required. Go to Project Settings > Work Items > Fields.
There you will see each of the Field Configurations in use in your project and the work items types that use them. Here I have two Field Configurations; one used by the Story work item type and the other used by all the other work item types.
If you click on the > character to the left of the Field Configuration name that will show you the list of fields in that configuration including a column to identify the ones marked as Required.

Our form requires a lot of 'Required fields' - and the error doesn't give a specific field that is causing an error. Is there a way I can contact you to resolve?

If I change the work request type to a Portfolio Epic instead of a Task, it works... (but I need it to be a task)

Here is a screenshot of the error - once I link a field to labels, a new error appears for components, and then more error messages. The form was working fine yesterday and nothing was changed... so it seems it might be due to a jira update?
