Error from Slack [500]

I have set up an automation on JIRA, using webhook generated from Slack, but it keeps giving Error 500 from Slack. Anyone able to advise or point me to the right direction?
Have followed the step-by-step guide on setting up the incoming webhook URL.

hi Nic! by that you mean setting up the webhook integration settings? then yes, else there's other settings that i need to look into?
