Dynamic Forms for JIRA - CLOUD

I can't seem to find any information on using the DFJ app once we move to the Cloud version. If not this application, what are some other options for creating dynamic forms for either SW or SC projects' issue types?t

I have researched ProForma (not cloud supported, nor is it exactly what I want, which is either converting dynamic PDF forms into a JIRA intake form, or creating dynamic forms within our JIRA project.) What are the CLOUD options for creating dynamic, cascading forms beyond these two applications?

As far as I can tell, Proforma vendor Thinktilt has been acquired by Atlassian and this is now a standard service management feature. Anyone able to confirm that?

Thanks Tom! I am new to JIRA project management, and am unfamiliar with the "standard service management" feature. Does this mean it comes with the our contract, or it is supported as an add on? Once someone is able to confirm that anyway. Thanks again!
