We add Screen copy for the discussion. After add another screen. When save - we see the same both pictures as last one. Please, see the file below. I can't add text document whic is much easy to see - use the photo here... Kind regrads, Alexander Goldberg

This issue happens because the file name is the same and most of the times, customers paste image/attachments directly to the body of the mail instead of saving and then attaching on the email/ticket.
We had a bug about this, but our developers closed it because it was not considered as a bug:

Hi all, this behaviour is certainly not a bug because it is the underlying JIRA platform behaviour for attachments which JSD is built upon, however it is a very valid suggestion especially for the portal use case.
There already exists a suggestion ticket: https://jira.atlassian.com/browse/JSD-4100 which I will recommend following for the latest update on the attachment behaviour improvements.
We have a feature request to implement the ability to do not show the same thumbnail when images have the same name because even though the image seems the same when opening the image it shows the correct one:
