Create a issue type Field

I have issue type bug field and sub task custom  field That have different work flows. 

I am getting below error on the edit and transition screens. I tried all sorts of configuration but it did not work. I would like to  display the name of the Issue type with its avatar on the screens. How do I do it using custom Fields. Please advice.
