Can we create a client automatically when we create an issue?

Agents can create customer accounts automatically when they create an issue for them. See our documentation on Raising requests on behalf of customers
If your Service Desk is set up to allow it Customer can also create accounts when creating a ticket for the first time, see Set up customer permissions
