Create a Ticket from a Table

So I am not sure how any of this would work. Here is what I would like to do:

 

  1. Material needed in production
  2. Employee scans a bar-code (unique identifier)
  3. The submit the ticket and the ticket fills in with information from a table
  4. The table should only be editable by a group of employees
  5. There may be up to 20 different fields, but they will all relate back to the original identifier
  6. I would also like to be able to use JIRA to print out the slips with the barcode and title and description

 

 

I would like to be able to complete this in JIRA Service Desk. Therefore, someone would submit a "ticket" which would create the issue and one of two things could happen:

 

  1. Automation would create a subtask for a certain group of employees to send out an alert.
  2. The "ticket" fills in extra fields that are not seen by the original employee from a separate table.

Any ideas?
