Could I create a Form that depend of the filter selected goes to a different Project?

I created in a SharePoint page a JIRA Form using a JIRA Collector script.

When user finished the request, it is go directly to a my JIRA backlog.

I will start working with another team.

My question here is:

For example, could I add some trigger or filters and to be redirect to more than one projects on JIRA?
