Hi Vera,

No, there is no out of the box capability for this. However, you can try one of the Atlassian Marketplace apps to get this functionality.

Hi @Vera Valshonok 

You may try an option mentioned Rilwan, but despite of these option there is no native way to include image or logo inside form. You may try Smart Forms for Jira app from my team, that supports branding request forms customization (like logo addition, colors customization, font changes). 

Smart Forms also supports:

- public sharing

- website embedding

- Confluence embedding

- custom confirmation messages with form answers and Jira issues reference 

- branded intake experiences

- work items creation 

and more

Hi @Vera Valshonok ,

Go to JSM Project settings --> Request Management --> Request Type.
Click of your Request type group and click on Request type

Once the Request Form is opened, click on Icon and Upload the required image.
