Can I set up a form on Jira with another form embedded, dependant on multiple choice answers?

Want to set up a form which has some generic questions but then redirects the submitter to another form dependant on answers. Is this possible?

Hi Monazzah, 

In my instance we have it set up so that after the user answers certain questions, it will either allow them to continue to submit a ticket or and info box will appear, letting them know the hyperlinked form they are looking for to fulfill their request. One thing it would be good to keep in mind is the user experience of:

1. Too many questions 
or
2. Too many redirects

To solve this, the first question we ask is typically a break fix or feature request question followed by a product/software/service question and then from there, we redirect the user to the correct Form if theyre not on it.
