Bulk Import of CSV - not working

Hi Team, 

I am trying to Bulk upload a sample CSV file to add issues to my Jira . However I keep getting the errors like this attached  I followed the procedure as per  this post :

My error file attached.  the CSV file I used is the exact format shared in above post
