Best practices for nextgen csv imports and properly linking epics, stories/tasks, and subtasks

I've read many, many threads on this topic but I'm not having any luck on getting the csv import to work properly in nextgen projects.

I have a super simple import that I'm testing with but my issues keep getting imported as stories. I don't mind if they're imported as tasks or stories at this point but the most important part is that I can import with parent/child relationships between them.

I'm also having troubles getting epics to import but those are so few that I'm fine with adding those manually for the time being. What am I doing wrong here?
