Best practice for Release management at test phase level

We need to have release note for every test phase level which is my SIT , UAT & Production. Should I create a version for each of these phases. or is there a best practice for accommodating this scenario. because with respect to time No. of releases may increase a lot. 

Another scenario is creating components. which approach is better or if we have any other approach to this
