Auto Add Tasks to an Epic based on a Custom Field Value

Hello, 

I am working on an automation to add extra tasks based on a custom field value. If the custom field filled as "YES", then I need the automation to create extra child tasks for this epic. Here is my not working automation below. Thank you!

What is not working with the rule?

Can you share the audit log?

Here is the audit log below, and this task issue has a subtask associated with it. Do I need to add additional steps just incase there are subtasks so the automation does not try to match them?

It’s going to run every time an issue is created. If the issue is not an epic, you’ll just get a message like you got in the audit log. This is not an error nor is there a problem with the rule. It just doesn’t match the condition. 

What is it that you were looking to happen?

There is a custom field called is person a manager? , if the answer is yes during the ticket creation; I would like the automation to add 3-4 additional tasks to this epic.

@John Funk , the automation rule exactly the same. All I had to do is to add epic name field to the create issue screen. Thanks.
